Senior Social Worker - Community Based Outpatient Clinic
The incumbent serves as a Senior Social Worker at the VA Diamond View Outpatient Clinic in Susanville, CA.
The incumbent works as part of the treatment team to provide both medical social work services and mental health services to the Veteran population at the clinic.
The incumbent is a member of the Patient Aligned Care Team (PACT) and also a member of the Mental Health Commuinty Based Outpatient Clinic (CBOC) / TeleMental Health (TMH) team within Mental Health Service.
The PACT is a comprehensive team which delivers primary care to Veteran patients in a longitudinal rather than episodic fashion, which focuses on prevention, health promotion, coordination and chronic disease management.
The incumbent provides various mental health services including mental health intakes, individual and group psychotherapy, and ongoing risk assessments for suicide.
The incumbent independently provides clinical psychosocial and case management services at an advanced practice level.
